申請に際しての注意事項

＜ユーザ名とメールアドレスの注意＞
「システムのネーミングに関する要領（http://www.tufs.ac.jp/common/icc/rule/iccuseofsystemnaming.html）」に従い，申請してください．また，以下の点についても注意してください．

・使える文字：「a～z（小文字のみ）」，「0～9」，「.（ドット）」，「-（ハイフン）」，「_（アンダーバー）」
　　　※大文字アルファベットは使えません．
　　　※数字を先頭文字にすることはできません．
・字数制限：6文字以上，32文字以下
・ユーザ名には「単語」や「姓名のうちどちらかのみ」など，推測しやすいものは使用できません．
・セキュリティ上の理由から，ユーザ名とメールアドレスを同じにすることはできません．必ず違う名前で申請して下さい．．

＜Notification for the user name and email address>
Please submit your application in accordance with the guidelines for naming the system (http://www.tufs.ac.jp/common/icc/rule/iccuseofsystemnaming.html).
In addition, please note the following points

Characters that can be used: "a to z (lowercase only)", "0 to 9", ". (dot), - (hyphen), and _ (underbar)
　　　You cannot use capital letters.
　　　You cannot use a number as the first letter.
Character limit: 6 characters or more, 32 characters or less
User names cannot be easily guessed, such as "words" or "only one of the family names".
For security reasons, please use a different user name and e-mail address.

注意事項・Notification

• 本申請書を提出することで「総合情報コラボレーションセンター利用細則」の内容を理解し，承認したものとします．
• 複数の利用者アカウントを取得することはできません．
• 利用者アカウント交付は原則 月曜日締めの金曜日発行となります．このフォームに入力した「連絡先メールアドレス」宛に交付書をダウンロードするURLをお送りします。
• 本人確認の為、Zoomを使用して身分証を提示していただく場合があります。

- By submitting this application form, you are deemed to have understood and approved the contents of the Rules for Information Collaboration Center.
- It is not possible to have multiple user accounts.
- It will take several days to proceed your application. And we will send you a URL to download the grant form to the email address you have submitted.
- You may be asked to show your ID using Zoom to confirm your identity.
