授業料免除申請及び徴収猶予を希望する方は、こちらのフォームに回答してください（一次申請（ウェブ入力））。
回答者に対してのみ、後日学務情報システムのメッセージにより、二次申請（申請書類提出）用の授業料免除申請書の様式を送付します。

今回のフォーム回答（一次申請）により、申請する学期の授業料の徴収猶予対象とし、授業料の自動引き落としを停止します。
授業料免除については、今後学務情報システムのメッセージにより送付する申請書の提出者に対し審査を行い、免除の是非を決定します（二次申請）。
With this form response (primary application), you will be eligible for tuition fee deferment for the semester for which you are applying, and automatic tuition fee withdrawal will be suspended.
The tuition fee exemption will be reviewed and a decision will be made on whether or not the applicant is eligible for the exemption.

①やむを得ない事情（unavoidable circumstances ; Your tuition payer died during the previous semester. The disaster struck during the previous semester.）
②経済的理由（economic reasons）
③授業料後払い制度に申し込むため
④コロナ家計急変（Sudden change in household finances due to coronavirus）
⑤授業料徴収猶予（tuition deferment）

※各制度の申請要件についてはホームページから確認してください。いずれの制度の要件を満たさずに一次申請を行い、二次申請書類の提出をした場合、審査不能となります。なお、審査不能について、二次申請書類の審査期間中に個別に通知することはありません。審査結果通知時に、他の不採用者と同じ通知を行います。
